The blood which is pumped by the heart flows through the arteries into the capillaries and then into the veins. The blood which carries glucose, oxygen and other nutrients supplies these things to the tissues of the body through the capillaries. Arteriovenous Fistula is an abnormality in which there is a direct connection between the artery and the vein, bypassing the capillaries. In this condition the tissues underlying the capillaries do not get enough nutrients and get damaged in the process.
AV Fistulas can be formed in any part of the body like legs, arms, lungs, kidneys or the brain. The AV Fistulas which are found in the brain or the spine are called Dural AV Fistula. The symptoms of dural AV Fistula can be sudden and severe headache, nausea, vomiting, weakness or numbness on one side of the body, loss of vision and body imbalance. In such situations one must consult a neurosurgeon without any delay. The complications of untreated AV Fistula can be bleeding in the brain which may cause irreversible neurological deficit.
Your neurosurgeon will advice an MRI Scan of the brain or the spine to diagnose Dural AV Fistula. MRI gives excellent information about the shape and extent of the dural AV Fistula and also shows any microbleeds. In the absence of MRI, a non-contrast CT Scan of the brain can also give useful information about AV Fistula in the brain or spine. The investigation of choice however is Cerebral Angiography in which a catheter is inserted from the femoral artery in the leg and is pushed right upto the carotid arteries in the neck and further. This test is done on a Cathlab and is therefore not easily available in many cities.
The treatment for Dural AV Fistula is to block the passage between the artery and the vein and this can be done either through endovascular coiling or through surgery. Endovascular coiling is a minimally invasive procedure and is done on a cathlab. Alternatively Stereotactic Radiosurgery using focussed radiation can be done to block this passage. This is a non-invasive procedure done in OPD setting but requires a Linear Accelerator.
